PCN TEAM

In addition to the Practice Staff , we have PCN staff  offering clinical care and support to our patients.  Prima Care Surgeries is part of  Shelton & Hanley Primary Care Network (PCN)

 

• Care Coordinators:   Amanda Thorley who works every day. She helps patients navigate the healthcare system and ensure they receive the right care at the right time. 
• Clinical Pharmacists and Pharmacy Technicians: 
•  David Kerrigan  : who works on tuesday
•  Delia  Mercas: works on Thursday
•  Iona Tudor : works on wednesday and Friday
•  They  provide medication management and advice to patients. 
• Physiotherapists: Louis Maxwell  works on Monday  Provide rehabilitation and support for patients with musculoskeletal issues. 
• Social Prescribers:     Clare Roberts: Help patients connect with community groups and activities to improve their well-being. 
• Mental Health Practitioners:   Deborah Ali  who Provide mental health support and interventions to patients.
